Advance Medical Directive
I don't want CPR,
intubation, or being kept alive by machines.
An Advance Medical Directive (AMD) lets you document your end-of-life wishes — signed at home in about 30 minutes by an MCHK-registered doctor.

HONEST & CLEAR
If my heart stops at home, will paramedics NOT do CPR?
AMD and paramedic CPR
An AMD does not prevent paramedics from performing CPR. They cannot determine on-site whether your AMD conditions are met. For that, you need a DNACPR order signed by two doctors.
An AMD is your advance declaration of wishes — it takes effect once a doctor confirms you meet the criteria. We recommend understanding both AMD and DNACPR.
We can help arrange both documents so your wishes are respected at home or in hospital.
Planning a home death? You may not need AMD or DNACPR
For a planned home death, a private doctor can issue a Medical Certificate of Cause of Death — no police, ambulance, or hospital. In that case, AMD and DNACPR are not required.
AMD and DNACPR matter most inside the public hospital system. If your plan is a home death, their role is different.
